Picture: A picture of your character.
Name: The name of your character.
Alias: Any nicknames your character might have.
Visual/Suggested Age: What age your character seems to be.
Actual Age: Your characters actual age.
Gender: What's your characters gender.
Looks: Despite the fact that you have a picture you need to fill this out.
Division: Leave this blank, an admin will tell you what rank and division your character is in based on its personality, abilities, and how well constructed the profile is. If your character is level 1, simply put Academy, as all characters beginning at level 1 must start there.

Personality: Your characters personality. This needs to be at least one paragraph, a paragraph being 5 lines across the page. Include good qualities, bad qualities, weaknesses, etc.
Likes: What your character likes.
Dislikes: What your character dislikes.
Code: What's your characters morals, what do they live by or live for.
History: Your characters history. Everyone needs to do this and it needs to be a minimum of 3 paragraphs, a paragraph being at least 5 lines across the page.
Family: Any family your character might have, living or deceased.
Loyalties: Who your character is loyal to.
Personal Items: Any item your character carries on their person.

Kidou:

Zanpaktou Name: The name of your Zanpaktou. Just the Japanese Name.
Zanpaktou Meaning: The English translation of your zanpaktou.
Zanpaktou Spirit: A picture and description of your zanpaktou spirit.
Zanpaktou Realm: A picture and description of your zanpaktou realm.
Zanpakuto Type: It will either be an elemental (if so simply state which element), Melee or Unclassified. We will rarely except zanpaktou with two elements unless the elements are linked in some way. However even if they are, it doesnt guarantee that the Zanpaktou will be approved.
Zanpakuto Shikai Release Form: In this section please state your weapon's release incantation. Then you state any changes in the weapon and then list its abilities.
Zanpakuto Bankai Release Form: Name the weapon's Bankai name if you have Bankai. Name any changes to the weapon and then state its abilities.
